Polycystic ovary syndrome (PCOS) is one of the main causes of female infertility. It affects 5-10% of women of reproductive age. Irregular menstrual cycle, insulin resistance and obesity are the key factors associated with PCOS. The relationship between various clinical parameters with PCOS is studied in detail. Various hormonal abnormalities like leutinising hormone, follicle stimulating hormone, prolactin, testosterone and estrogen levels are analyzed. The lipid parameters like low-density lipoprotein, high-density lipoprotein, very low-density lipoprotein and total cholesterol are also studied along with insulin, blood glucose and glucose challenge test. This book highlights the diagnostic utility of various biochemical parameters and the role of metformin as an insulin sensitizing agent in the treatment of PCOS.
The aim of this work is to conduct a systemic overview and critical analysis of the body of data in the literature on polycystic ovary syndrome (PCOS). PCOS is the commonest single cause of anovulation; affecting approximately 5-10% of women in the reproductive age. The syndrome can present in a variety of clinical manifestations including oligomenorrhea, hirsutism, obesity, and infertility. Endocrinal features include high sustained level of luteinizing hormone with normal level of follicle stimulating hormone, and hyperandrogenism. PCOS may be also associated with hyperinsulinemia, glucose intolerance, and altered lipid profile. Medical induction of ovulation is considered the first line of treatment for anovulatory PCOS. Laparoscopic ovarian drilling may be of value in women resistant to medical induction. However, the efficacy and long-term safety this technique is not yet established.
Acne is one of the major manifestations in patient with PCOS. Although acne is detected only in about 23-35% of patients with PCOS, some studies show a high prevalence of PCOS in acne patients. Acne has an overall prevalence in women of about 12%, tends to persist till menopause, and then declines with age.Hyperinsulinemia, insulin resistance and hyperandrogenemia are some endocrine findings in adults with PCOS.Hyperandrogenism characterized clinically as Acne, Hirsutism, Androgenic alopecia and/or chronic anovulation in the absence of specific adrenal and/or pituitary disease. Clinically, diagnosed woman with PCOS has higher risk for infertility, dysfunctional bleeding, endometrial carcinoma, obesity, type 2 diabetes mellitus, dyslipidemia, hypertension, and possibly cardiovascular disease. So to prevent risk factors associated with PCOS,it has to be diagnosed and treated earlier. Female with Acne are unaware that they may suffer from PCOS. So Acne patients coming to dermatologist must be diagnosed for PCOS,so that they can be treated at early stage and risk factors of PCOS can be prevented.
In view of the increasing number of polycystic ovary syndrome (PCOS) among the Sudanese women, and its sequences, such as infertility. This study was conducted to investigate the biochemical changes occurring in the follicular fluid associated with PCOS, which may help in the diagnosis of this disease. This study was carried in the department of biochemistry, Faculty of Medicine in the National Ribat University, in collaboration with Reproductive Health Care Center (RHCC), Sudan Assisted Reproductive Center (SARC) and Asia Hospital in Omdurman. The study was carried out during the period: July 2004- May 2008. A group of 111 women were enrolled in this study; 55 of them were PCOS cases and 56 of them had normal ovaries. Those with normal ovaries represented the control group. The study included women from different Sudanese states; some of them were housewives, while others were employees. The different paramertes measured in PCOS patients were compared with those measured in control group. Six hormones in the follicular fluid were measured by automated chemical analyzer (Immulite 2000).

This work is a clinical and genetic analysis of the autosomal Alport syndrome and collagen IV nephropathy (alpha 3/alpha 4). It contains the description of three new pathogenic mutations which lead to autosomal dominant Alport syndrome: one in the COL4A3 gene and the other two in the COL4A4 gene. It also contains the study of the clinical course of the autosomal dominant Alport syndrome in a spanish population, which is as agressive as the one in the other paths of inheritance.
